Advanced Heart Failure Program Services

Tailoring your treatment to your individual needs

The Heart Failure Clinic at Orlando Health Heart & Vascular Institute offers care to patients who have recently been hospitalized for acute (sudden) heart failure. We also offer same-day or next-day appointments to quickly address acute (sudden) issues that do not require hospitalization.

For patients who come to us following hospitalization, you will consult with a physician and pharmacist in our Heart Failure Clinic within seven days of leaving the hospital. Our team members will tell you about your medication regimen and any necessary follow-up care during this visit. Your physician will also evaluate your heart health and update your recommended treatment plan if needed.

OUR HEART FAILURE SERVICES

To help you stay as healthy as possible, our Heart Failure Clinic team offers a variety of services to care for all your medical needs:

Hospice care

Our compassionate team members assist in finding and providing care for conditions that are ultimately life-limiting.

Outpatient diuretic clinic

Diuretics help many people with heart failure manage symptoms and slow disease progression. We offer intravenous (IV) diuretic therapy in an outpatient setting to help treat acute (sudden) heart failure exacerbations (worsening of symptoms) that don’t require an overnight stay in the hospital.

Palliative/supportive care

Our palliative care team members help soothe your pain and manage your heart failure symptoms. Palliative care offers you a better quality of life while living with heart failure.

REACH team

Our specialized social work team helps you find valuable community resources and connects you with appropriate follow-up care.
